On admission, the patient was hypercalcemic to 14.6 mg/dL and hypokalemic to 2.6 mg/dL. According to his wife, the patient's mentation and oral intake had been declining, with worsening abdominal pain, fatigue, and vomiting after eating. On physical examination, the patient appeared cachectic and uncomfortable, with tenderness to palpation in the right upper quadrant.
