Biography
Dr. Fazel is an assistant professor at the University of Arizona (UA) College of Pharmacy. She is a clinical pharmacy specialist in ambulatory care and an American Diabetes Association (ADA) recognized Adult Diabetes Self-Management Education (DSME) program coordinator at Banner – University Medical Center South Campus, Diabetes Physician Offices at Abrams and Diabetes Prevention and Education Center (DPEC).
Dr. Fazel is a Board Certified Pharmacotherapy Specialist, a Board Certified Ambulatory Care Pharmacist, and Certified Diabetes Educator. Dr. Fazel practices as part of an interprofessional team that provides diabetes management and education.
Dr. Fazel received her doctor of pharmacy degree from the UA College of Pharmacy. She completed a PGY1 pharmacy practice residency at the Southern Arizona Veteran Affairs Health Care System.
Prior to joining the University of Arizona, Dr. Fazel practiced in various areas of pharmacy including the community setting and as a specialist in poison information at the Arizona Poison and Drug Information Center.
Dr. Fazel’s research areas of interest are health outcomes, diabetes prevention/education, interprofessional practice and education.
